$40-$45 per hour | $1,000 signing Bonus.

JOB DESCRIPTION:
A Behavioral Health Specialist within the Enhanced Care Management (ECM) Program at Mission Pediatrics plays a crucial role in providing high-quality care management to improve the overall health and well-being of pediatric members and their families. Operating under the general supervision of the Enhanced Care Management Department Leadership, you will collaborate within the ECM team, engaging in therapeutic interactions, coordinating care, and ensuring seamless transitions for member with behavioral health needs.

TASKS:

  • Responsible for managing a caseload of members with behavioral health needs (primarily of Tier 1 or Tier 2).
  • Responsible for utilizing brief behavioral health therapeutic interventions to improve the member’s ability to manage their own health.
  • Responsible for brief crisis interventions and warm hand-offs to local crisis resources as needed to address behavioral health needs. Provides members and family education about chronic medical and behavioral health conditions to improve health literacy.
  • Provides formal and informal training and support for ECM members on behavioral health conditions, including treatments and evidence-base for treatment.
  • Assists with the coordination of medical and behavioral health access issues with PCP offices, specialists, and ancillary services.
  • Tracks medical and behavioral health outcome measures in a patient registry.
  • Ensures smooth transitions of care, coordination with psychiatric hospitals for M1 and with IEHP transitions of care team for M2, regarding client admission/discharges.
  • Collaborates on member care issues with other ECM care team members, participating in weekly systematic case reviews and ad hoc case reviews.
  • Works with members to identify health/wellness goals and incorporates these goals into the member’s shared care plan that facilitates communication among members and providers.
  • Aid Nurse Care Manager in overall review of member comprehensive health assessments (CHA) upon completion by another care team member.
  • Responsible for ongoing engagement with members, both in-person and on the phone, in a manner that utilizes evidence-based approaches (such as motivational interviewing) that promotes collaboration between the member and his or her medical/behavioral team, as well as to increase the member’s sense of control over their whole health.
  • Provides consultation for all members of the team when discussing tasks of a clinical nature.
  • Demonstrate the highest ethical behavior in relationships with co-workers, supervisors, members, providers, and community colleagues.
  • Exhibit dedication to continuous quality improvement by actively participating in initiatives and projects.
  • Ensure accurate documentation, adhering to regulatory requirements and accreditation standards.

EDUCATION REQUIREMENTS:

  • Minimum Education: Master’s degree in social work or Marriage and Family Therapy (MSW or MFT) from an accredited institution.
  • License: Possession of an active, unrestricted, and unencumbered Clinical Social Worker (LCSW) or Marriage and Family Therapist (LMFT) license issued by the California Board of Behavioral Sciences. Registered with the Board of Behavioral Sciences is required.

SKILLS AND CHARACTERISTICS:

  • Experience working successfully within a team is mandatory.
  • Familiarity with Managed Care and discharge planning is preferred.
  • Highly skilled interpersonally, with excellent relationship and conflict resolution skills.

EXPERIENCE REQUIREMENTS:

  • Minimum of one (1) year of experience performing or facilitating Behavioral Health services in a health clinic/psychiatric hospital medical facility.
