Weekend Start of Care (SOC) Nurse (RN) – Full-time

Qualifications

Required:

  • Associate degree or higher
  • Valid Certified Cardiac Rehabilitation Professional (CCRP)
  • Valid Registered Nurse (RN) in the state of Illinois (IL)
  • Valid Regular Driver’s License (DL)
  • ALL of the following valid licenses/certifications:
    • American Heart Association Basic Life Support (AHA BLS)
    • American Heart Association Advanced Cardiac Life Support (AHA ACLS)

Overview

Our Award Winning Client is seeking a Full-Time Weekend Start of Care (SOC) Nurse (RN) to promote the delivery of coordinated, comprehensive care to patients through assessment, planning, documentation, and provision of direct nursing care. The ideal candidate will have an Associate Degree in Nursing, at least two years of RN experience, and be CPR-certified. Responsibilities include accurately completing assessments, administering skilled nursing care, and collaborating with other healthcare professionals to ensure patient care needs are met.

Key Responsibilities

  • Accurately complete required start-of-care documentation on new admissions, post-hospitalization.
  • Assess the patient for home telemonitoring, incorporate telehealth into the plan of care, and ensure notification of patient admission to the telehealth program within established timeframes.
  • Develop and implement the patient care plan in cooperation with other Agency professionals and paraprofessionals who will be providing care to the patient.
  • Observe signs and symptoms and report to the physician reactions to treatments, including drugs and changes in the patient's physical and emotional condition.
  • Teach, supervise, and counsel the patient and family members regarding nursing care needs and initiate preventive and rehabilitative nursing procedures as appropriate for the patient's care and safety.
  • Assess and assign Home Health Aides as appropriate by providing a written plan of care and specific duties the Aide is to perform for each individual patient.
  • Initiate preventive and rehabilitative nursing procedures as appropriate for the patient’s care and safety, according to the Agency’s policies and procedures.
  • Communicate with a supervisor and, as indicated, with other members of the home care team concerning patient problems in a timely manner, using current processes and technology available to the organization.
  • Attend and participate in office staff meetings, patient care conferences, record audits, utilization reviews, performance improvement, in-services, workshops, and seminars.
  • Satisfy CEU requirements of the State Board of Nursing.
  • Stay up-to-date on reimbursement criteria and charting needs for all patients.
  • Strive to improve nursing care through continuing education, attendance at workshops and conferences, active participation in professional and related organizations, and individual research and readings.
  • Adhere to the Agency’s personnel policies and Compliance Plan.
  • Display a high degree of courtesy, tact, and knowledge of services provided by the agency in all interactions with staff, patients, and visitors.
  • Respect patient rights and property.
  • Must call in reports daily to a Supervisor or scheduler.
  • Contact the Nurse Supervisor to inform them of any alternate schedules for patient visits during vacation, sick days, or personal days. Immediately report any schedule changes or inability to complete assignments as scheduled.
  • Accept schedule changes/additions during staffing difficulties when necessary.
  • Organize daily visit schedules to minimize travel time and mileage. Maintain productivity according to work status. Travel may be required to meet weekly productivity goals.
  • Meet weekly productivity points per week—equivalent to three (3) SOCs or ROCs per day, for a total of six (6) points.
  • Work holidays as required.
  • Timely and accurate documentation is required; untimely or poor documentation is subject to disciplinary action, up to and including termination.
  • Travel as necessary to meet business needs. Our Award Winning Client will always do their best to keep clinicians within a reasonable territory.
  • Perform other duties as assigned by the supervisor.

Schedule and Shift Details

  • Schedule: Every Saturday & Sunday + two weekdays for four 10-hour days; preference for Friday – Monday or Saturday – Tuesday

Travel

  • Must travel to locations as needed to meet productivity requirements. Our Award Winning Client will always do their best to keep clinicians within a reasonable territory.

Home Health RN, Start of Care, Weekends – $10,000 Bonus

  • Compensation: $80,000 – $90,000 (salaried)
  • Full-time: 40 hours per week; 28 points per week
  • Schedule: every Saturday & Sunday + two weekdays for four 10-hour days; prefers Friday – Monday or Saturday – Tuesday
  • Coverage area: Chicago, Evanston, Skokie, Glenview, Park Ridge & surrounding (larger coverage area since they’re only completing SOCs)
  • Requirements:
  • Must have an IL RN license
    • Minimum of 1 year of licensed RN experience in a hospital setting (med-surg, telemetry, ER, or ICU preferred)
    • 2 years of home health experience
  • Orientation Notes: Must be available to orient Monday – Friday, 8-hour days for at least the first two weeks
