Job Description

Qualifications:

• Experience in home infusion of patients with chronic illnesses within the comfort of the patients' homes, Ambulatory Infusion Suites, and/or Clinics.
• Ability to work independently in the field.
• Ability to provide patient education in accordance with the physician’s plan of treatment.
• Ability to Initiate, maintain, and discontinue peripheral IV lines.
• Ability to access and deaccess Implanted Ports.
• Ability to assess patient condition before, during, and after infusion therapies and utilizing critical thinking and nursing judgement skills.
• Ability to effectively assess IV sites, Implanted Ports, and other Central lines for signs of infection, phlebitis, infiltration, and/or catheter dislodgement.
• Ability to monitor patients for signs and symptoms of infusion reactions and overall response to infusion therapy.

Duties and Responsibilities:

• Coordination with the patient, physician, pharmacist and other multidisciplinary teams to ensure effective continuity of patient care.
• Providing telephonic and face-to-face patient educational services.
• Keeping current regarding community health issues and regulations.
• Coordinating lab draws and delivery per physician's plan of treatment.
• Documenting all assessments and communications that pertains to patient care
