Key Responsibilities

1. Provide direct patient care in a long-term care environment, including administering medications and managing care plans.
2. Monitor and document patient progress, identifying any changes in condition.
3. Collaborate with interdisciplinary teams to ensure the best outcomes for residents.
4. Support daily activities for residents, fostering a safe and compassionate environment.
5. Communicate effectively with residents, families, and healthcare teams.

Qualifications

1. Active RN license in the state of assignment.
2. Minimum of 1-2 years of nursing experience in a long-term care setting preferred.
3. Proficiency with Epic EMR system is a plus.
4. BLS certification required; additional certifications in geriatric care are advantageous.
