We are expanding and seeking a dedicated individual to join our dynamic team for a hybrid position in the area, offering a hybrid onsite and remote opportunity tailored to fit your schedule.

Position Highlights:

• Work Schedule:  20 hours per week with approximately 20% completed remotely via telehealth. Two days per week are onsite and a half day per week is remote.
  
• Compensation:  $40.00-$47.00 per hour, based on experience.
  
• Travel Compensation:   Paid at your hourly rate for travel time to and from facilities.
  

Qualifications & Experience:

• Registered by the Commission on Dietetic Registration.
  
• Minimum of 1 year clinical experience (preferred).
  
• Proficient in providing general nutrition interventions.
  
• Experience with tube feedings.
  
• Familiarity with electronic charting systems.
  
• Knowledgeable about state survey process and compliance to nutrition care regulations.
  
• Excellent oral and written communication skills.
  

Primary Responsibilities:

• Conduct comprehensive nutritional assessments for patients and residents.
  
• Administer medical nutrition therapy, including for wound healing and weight management.
  
• Provide nutritional counseling and education to residents and families.
  
• Ensure compliance with DOH/state/federal regulations and company standards.
  
• Work independently with minimal supervision.
